- Full legal name — as it appears on your government-issued ID
- Date of birth — required for identity verification and background check submission
- Fingerprints — digital or ink-captured biometric data for criminal history checks
- Social Security Number — required by FDLE, FBI, and certain licensing agencies for accurate record matching
- Residential address — for identification and mailing of apostille documents
- Payment information — processed securely through our payment processor; we do not store full credit card numbers
2. How We Use Your Data
Your personal data is used exclusively for the purpose of completing your requested background check or document authentication service:
- Electronic submission of fingerprints to FDLE and/or FBI CJIS systems
- Generation of FD-258 fingerprint cards for agency submission
- Apostille and authentication processing through the Florida Secretary of State or U.S. Department of State
- Communication regarding your appointment, results, or order status
- Compliance with applicable laws and legal obligations
We do not sell, rent, trade, or use your data for marketing, profiling, or any purpose other than completing your requested service.
3. Data Retention Policy
Provn retains your personal data only as long as necessary:
- Fingerprint images: Deleted from our systems within 30 days of successful transmission to FDLE/FBI, or immediately upon your written request after results are received.
- Personal identifiers (name, DOB, SSN): Retained for 90 days after service completion to handle any resubmission, rejection, or customer support needs, then securely purged.
- Transaction records: Retained for the period required by applicable law and our business records policy (typically 3–7 years).
- Payment records: Retained per standard accounting and tax requirements.
4. Third-Party Sharing
Your data is shared only with the government agencies necessary to complete your requested service:
- Florida Department of Law Enforcement (FDLE) — for Live Scan background checks
- Federal Bureau of Investigation (FBI) — for Identity History Summary checks
- Requesting licensing agencies — e.g., AHCA, Board of Nursing, DBPR, as specified by your ORI number
- Florida Secretary of State / U.S. Department of State — for apostille and authentication processing
We do not share your data with any other third parties unless required by law or court order.
5. Your Rights Under Florida Law
Under Florida's Information Protection Act (FIPA) and other applicable laws, you have the right to:
- Request a copy of the personal data we hold about you
- Request correction of inaccurate personal data
- Request deletion of your personal data (subject to legal retention requirements)
